Liability Waiver & Release

provided by Hand to Heart (“Company”), I acknowledge and agree as follows:

Educational Purpose Only

I understand that the content provided by Hand to Heart is for general educational and

informational purposes only. This course does not provide CPR, First Aid, BLS, or any other

certification, and does not replace hands-on training or evaluation by an authorized training

provider.

No Medical Advice or Provider Relationship

I acknowledge that participation in this course does not constitute medical advice and does not

create a nurse-patient, provider-patient, or professional relationship between myself and

Hand to Heart or its instructor(s), including any licensed professional associated with the course.

Assumption of Risk

I understand that performing CPR, choking-response techniques, or first aid involves inherent

risks, including the risk of serious injury or death. I voluntarily assume all risks associated with

viewing, learning, practicing, or attempting to apply the techniques demonstrated in this course.

Emergency Disclaimer

I understand that this course is not a substitute for emergency medical services. In an actual

emergency, I agree to call 911 or my local emergency number immediately.

Release of Liability

To the fullest extent permitted under Tennessee law, I hereby release, waive, and discharge Hand

to Heart, its owners, instructors, contractors, and affiliates from any and all claims, demands,

damages, losses, or causes of action arising out of or related to my participation in or use of this

course, including but not limited to claims based on negligence.

No Affiliation or Certification

I understand that Hand to Heart is not affiliated with, endorsed by, or sponsored by the

American Heart Association (AHA), and that no AHA materials, certifications, or credentials are

provided.

Voluntary Participation

I affirm that I am participating voluntarily, that I am legally competent to agree to this waiver, and

that I have read and understand this document in its entirety.

Governing Law

This Waiver & Release shall be governed by and interpreted in accordance with the laws of the

State of Tennessee.
